Apple Watch Ultra 4 is starting to sound like more than a yearly refresh, since the lineup has not changed at all since its launch. A new report claims that Apple is preparing a full redesign alongside a bigger health sensor upgrade for its next rugged smartwatch.

According to a new report from DigiTimes, Apple Watch Ultra 4 could feature a “full redesign” and a significant upgrade to its sensing functionalities. The report corroborates past supply chain claims suggesting that Apple was planning an exterior design shift, potentially including a new sensor layout on the back of the watch.

The bigger user-facing change could be blood pressure monitoring, but do not expect a tiny cuff on your wrist for obvious reasons. The reported feature would use the optical heart rate sensor to analyze how blood vessels respond to each heartbeat and alert users when an abnormal pattern is detected. The feature is reportedly under FDA review, which will possibly be approved ahead of the wearable’s launch.

There is one messy detail here which conflicts with an existing feature. Apple already introduced Hypertension Notifications with watchOS 26, which looks at blood vessel responses over longer periods. The new report does not make it completely clear how the Apple Watch Ultra 4 version would be different. From what we know, it may be a more refined or clinically validated version of the same underlying idea.

This Could Be Apple’s Real Garmin Problem, Not Just Another Bigger Screen

The interesting part is not the redesign though, it’s what the company is trying to make the Ultra become. For years, the Ultra has been the Apple Watch for people who want more battery, tougher material, and a more outdoorsy look, and if Apple adds a stronger health sensor, the Ultra 4 could move closer to being the serious health and performance watch in the lineup.

Market observers cited in the report believe that the redesign could lift Apple Watch shipments by 20 to 30 percent compared to 2025. This suggests that Apple may be treating this as a bigger cycle than usual. The Apple Watch Ultra 4 is expected to launch later this year in the fall of 2026 alongside the Apple Watch Series 12 and iPhone 18 Pro lineup.
